The effect of glycemia on the short-term prognosis in patients with type 2 diabetes mellitus hospitalized with decompensated chronic heart failure

Abstract

Aims - to assess the effectof admission glycemia on the short-term prognosis in patients with type 2 diabetes mellitus (T2DM) hospitalized for decompensation of chronic heart failure (CHF).

Material and methods. A retrospective analysis of the charts of 279 patients with T2DM hospitalized with CHF decompensation was performed.

Results and discussion. The hospital mortality rate for this sample was 6.0%. In patients who died in the hospital, the glycemia at admission was significantly higher [11.20 (8.25; 14.93) mmol/l] than in tho seprescribed [8.33 (6.47; 11.55) mmol/l] (p=0.016). Addmissionglycemia was a risk factor for hospital mortality, adjusted for kidney dysfunction, leukocyte count and anasarca. Severe clinical manifestations of CHF decompensation and kidney dysfunction were also independent risk factors for hospital death.

Conclusion. Admission glycemia can be considered as a modifiable risk factor for an unfavorable shortterm prognosis in patients hospitalized with CHF decompensation. Admission hyperglycemia is an independent factor determining the risk of hospital mortality.
