Sheehan syndrome in a patient with thyroid disease

Abstract

Sheehan's syndrome, first described by Harold Leeming Sheehan in 1937, is ischemic pituitary necrosis due to severe postpartum hemorrhage, characterized by impaired synthesis of pituitary hormones. It is known that an increasing of the pituitary gland occurs during pregnancy (up to 36%) due to hypertrophy of prolactin-secreting lactotrophs. It leads to the vulnerability of the pituitary gland to ischemia. This fact explains the absence of Sheehan's syndrome in non-pregnant. Depending on the number of affected cells and the hormones level, manifestations of the syndrome can vary from an affected single hormone secretion to the complete cessation of the functioning of the anterior pituitary gland, which can lead to coma and even death. The absence of lactation and the resumption of the menstrual cycle after childbirth, complicated by heavy bleeding, are the most important signs in the diagnosis of Sheehan's syndrome.
