Integrated evaluation possibilities of man’s hypothyroidism pathogenetic effect on erectile dysfunction forming

Abstract

The article is devoted to the thyroid gland pathology in the spreading of different sexual disorders in male population.

The aim of research – to study the occurrence frequency of decreased thyroid hormones level among the patients with erectile dysfunction (ED) and to assess the effect of hypothyroidism on hemodynamic measures of penile arterial and venous blood flow.

Material and methods. Research has been realized in the LLC “Andrologic center”, Voronezh, for three years. The research subjects were 350 male patients of in the age of 18 and 68 years old (middle age – 45.5±1.5year) with different manifestation of ED.

The results of physical examination, finger rectal examination, blood hormonal profile (total testosterone –tot.T, globulin that binds sex hormones – GBSH, prolactinium – PL, estradiol – E2, thyroid stimulating hormone –TSH, free thyroxine – T4) and also instrumental researches (ultrasound examination of the scrotum, transrectal ultrasound examination (TRUSE) of the prostate ultrasound dopplerography (USDG) of the penis’ vesicles within tracavernous injection of vasoactive drugs) have been evaluated.

Processing of statistical data and calculation of correlation coefficients have been accomplished by using analytical mathematical program Excel 2013 (Microsoft).

Results and discussion. Subclinical hypothyroidism has been identified in 16%, and manifest in 7.1%among patients with ED. The decreasing of tot.T has been revealed in 33.7% cases and such status was observed in 78.6% cases among the patients with detected hypothyroidism (r=0.92, р<0.01). Increased level of PL has been revealed among 6.0%, and the hyperprolactinemia frequency has achieved 32.1% (r=0.73, р<0.05) in the group of patients with hypothyroidism. In accordance of USDG results the features of venous and corporation ED have been revealed in 55.7% cases; the frequency of enhanced venous drainage occurrence has been stated in 76.8% (r=0.86, р<0.01). In accordance of TRUSE the occurrence of “flattened” form of prostate in hypothyroid group with hypogonadism traits was 81.8% (r=0.66, р<0.05) and in hypothyroid group as a whole – 58.9%.

Conclusion. Conducted research indicates the significant importance of thyroid hormones’ level in male reproductive health ensuring. Close relationship between hypothyroid state as prevailing factor, decreased sexual hormones level and penis vascular disorders severity has been convincingly proven in ED forming (r=0.895, р<0.05).
