Risk of testosterone deficiency in men with type 2 diabetes depending on the level of glycated hemoglobin

Abstract

Type 2 diabetes mellitus (T2DM) and androgen deficiency are diseases, the combination of which in the patient leads to a significant severity of each of them. There are no data on the critical values of HbA1c levels that provoke the development of testosterone (T) deficiency. Approaches to assessing in clinical practice the risk of T deficiency in men with T2DM haven’t accessed.

Purpose of the study – to evaluate the relationship between HbA1c level and the risk of T deficiency development in men with T2DM.

Material and methods. The study included 487 men with T2DM. Clinical, anthropometric and hemodynamic parameters, fasting glycemia levels, HbA1c, and total testosterone (T) were studied. Statistical methods included ROC analysis and logit regression method, carried out using the Statistica 10.0.

Results. The frequency of T deficiency in men with T2DM was 44.6%. The rise in the level of HbA1c more than 10% increases the risk of T deficiency development in patients with T2DM by 1.6 times (confidence interval 1.2–2.1, the area under the ROC-curve 0.629±0.052, p=0.013). A mathematical expression that describes the relationship between the risk of T deficiency development and the level of HbA1c in the blood was obtained (chi-square=5.1; p=0.023). This allows to assess the risk of T deficiency development in men with T2DM in clinical practice.

Conclusion. The increase in HbA1c level more than 10% increases the risk of T deficiency development in patients with T2DM by 1.6 times. Calculation of the T deficiency risk in men with T2DM has a great practical importance, allowing for early diagnosis and timely correction of hypogonadism.
